Interest

Donna uses specialized skills in assessment, planning and treatment to provide therapeutic interventions to prevent dysfunction, restore function, and/or reverse the progression of pathology of the upper limb in order to enhance an individual’s ability to execute tasks and to participate fully in life situations.

Donna grew up in Whatcom county where she still resides. She enjoys playing outside which includes biking, fishing, snowshoeing, hiking, and gardening. She grew up riding horses and competing. She still enjoys riding horses whenever she can.

Interest

Neil graduated from Western Michigan University with a Master of Science degree in Occupational Therapy in 1997 and is a certified hand therapist, which is an extensive multi-year professional certification process, to help ensure the highest quality of therapeutic intervention for upper extremity disorders. When not working at the clinic, Neil enjoys spending time with his wife and daughter, being active outdoors, and racing outrigger canoes.

Patient Intake

Patient Intake Form- Skagit Hand Therapy
I consent to occupational therapy treatment. I authorize mutual exchange of information between Skagit Hand Therapy and referring health care providers and insurance carriers concerning my injury/condition and treatments. I have the right to review Skagit Hand Therapy’s privacy notice, to request restrictions on Skagit Hand Therapy’s use and disclosure of healthcare information and to revoke this consent to release information. I assign all payments for occupational therapy services rendered to myself or dependents to Skagit Hand Therapy. I understand that I must check with my insurance provider to confirm specific terms of occupational therapy coverage. I accept responsibility for any services not covered by my insurance policy and for any services I choose not to submit to my insurance company. I understand that each visit for occupational therapy treatment without any insurance benefit is payable at the time of service. Cancellation and Missed Appointment Fees If you are unable to keep your appointment, we respectfully require 24 hours’ notice. A $25.00 fee may be charged for a “no show” appointment, to be collected on or before the next appointment. If a patient has three or more no-show appointments, they may not be allowed to hold future scheduled appointments and may only be allowed to make same-day appointments.
